If you want to practice in Texas, start thinking about the following 6 months before graduation:
1. Texas State License (3 months minimum)
2. DPS # (get this on same day that you arrive in Austin for your license interview)
3. DEA # (8 weeks, you may already have one if you completed a fellowship...but may need to have it re-registered)

*You may also need a Medicare UPIN #

You will need all of the above and possibly a face sheet of liability insurance in order to get hospital privileges and to listed on other insurance plans.

If you start working in Texas for an established practice, the office can switch your personal UPIN # to their office's #.

It will take 30 days to 6 months to get on some insurance plans & 30-90 days to get hospital privileges.

Thanks for the basic "must haves" list to practice medicine in Texas. I think the same standard procedure is applicable to other states. Some even have more requirements aside from what you had mentioned. Heart Surgery
